These plans are available using the Cigna national OA Plus network, the Anthem Blue Cross National Blue Card network and the new Northwell Health network. These PPO plans are available nationally. These plans all renew every January 1. These plans are great for the self-employed or groups, no group participation requirements and rolling enrollment all year. These plans use the national Anthem Blue Card PPO network, Cigna OAP network or the smaller Multiplan PHCS network or the NJ only Qualcare network. These plans renew 12 months after enrollment. Enrollment can be the 1st of the month for any month of the year. The deadline for enrollment is the 18th of the month prior to the effective date. Prices start at $607.00 per month.

These MEC plans are our lowest cost plans that includes hospital in-patient benefits and surgical benefits. These are $0 deductible plans with maximum visits per plan year for most services. Premiums start at $456.50 per month.
2. Oxford Health insurance for small business groups (View Quotes)
(a UnitedHealthcare company) EPO, PPO, and HSA plans are available for your employees. 70% of small businesses, in downstate NY, that offer health insurance to their employees, use Oxford health insurance. Oxford offers three networks to choose from, Freedom, Liberty and Metro. Freedom and Liberty plans gives your employees national coverage (with the UnitedHealthcare Choice Plus network for Freedom and the UnitedHealthcare Core network for Liberty). Oxford allows for class carveouts, as an example you can set up the company Executives on an Oxford Freedom plan and the staff on an Oxford Metro plan or on the MagnaCare association plan. Available in all downstate NY counties up through Ulster and Sullivan Counties. 4th quarter 2024 and 1st quarter 2025 rates posted.

Empire and Anthem will have plans available with the full EPO / PPO network, plans with the slightly smaller Blue Access network, and the smallest network called Connection. Empire and Anthem will be available for businesses located in New York City, Long Island, Mid-Hudson regions and the Albany region. All plans can be used nationally. All plans will not require referrals. Empire and Anthem will allow almost unlimited out-of-area employees to be enrolled. 4th quarter 2024 and 1st quarter 2025 rates are posted. 5. Multiplan PHCS plan (View Quotes)
Lowest cost and lowest participation requirement plans, to be compliant with large group penalties.
This plan is very low cost with EPO benefits for businesses with over 100 full time employees in NY with a minimum of 10 enrollments for the MEC plans and a minimum of 5 enrollments for the MV plans to qualify. All other states a minimum of 10 enrollments will qualify for any size business. This national PPO plan uses the Multiplan PHCS network in all 50 states.
These are the lowest cost plans that satisfy the mandate for businesses with 50 or more employees to satisfy the ACA part A and part B penalties. This plan requires 10 or more employees to be enrolled to qualify. The Choice MV plan at $595 per month is the lowest cost option to satisfy the part A and B penalties. If a business pays for the lowest cost plan ($54 per month for every full-time employee) both the part A and part B penalty's will not be incurred. This plan can also be back dated, if required, to save your business the penalty for the whole year or more.
6. Lifeline Medical Plus plan (View Quotes)
These plans are very low-cost Minimum Essential Coverage (MEC) plans. They Include Wellness & Preventative Care, Telemedicine, Primary Care office visits and specialist Office visits, Urgent Care visits, Lab Work, X-rays and prescription drug benefits. These plans do not include hospital benefits. These plans can be used to prevent the part A penalties for businesses of over 50 equivalent full-time employees.
